The concept of edema

The main clinical manifestations are edema of the eyelids, head and face, limbs, abdomen and back, and even the whole body. Severe cases may also be accompanied by pleural effusion and ascites.

The etiology and pathogenesis of edema

The pathogenesis of edema mainly lies in the dysfunction of the lungs, the spleen, the kidneys, and the triple burner gasification is unfavorable. The disease is located in the lungs, spleen, and kidneys, and the key lies in the kidneys.

Syndrome differentiation and treatment of edema

The syndrome differentiation of edema is based on yin and yang. First, Yang water and Yin water are distinguished, and their pathological attributes are distinguished. Yang water is mostly caused by wind evil, sores, and water dampness. The onset is more acute, usually occurring within a few days. The swelling usually starts on the face, goes from top to bottom, and then spreads to the whole body. The skin in the swollen area is stretched and shiny, and the depression appears when pressed. It also has superficial symptoms such as fever and aversion to cold; or Toxin-heat syndromes such as irritability, thirst, red and astringent urine, constipation, and skin sores are superficial and substantive syndromes, and generally have a short course.

The causes of yin water are mostly due to fatigue, spleen and kidney deficiency caused by congenital or acquired factors, and the onset is slow or repeated, or it is transformed from yang water. The swelling mostly starts from the ankle, goes from bottom to top, and then spreads to the whole body. The skin in the swollen area is loose, and the depression is difficult to recover when pressed, or even feels like mud when pressed. It is also accompanied by fatigue, lack of appetite, loose stools, soreness, coldness, and pain in the waist, and nausea. Cold limbs and other symptoms of spleen and kidney deficiency. If the condition is internal, empty or mixed with excess, the course of the disease will be longer.

For the treatment of edema, "Suwen: Decoction and Sauerkraut Theory" puts forward three basic principles: "Open the gate of ghosts, cleanse the house, and remove the flowers, leaves, and herbs."

(1) Yang Shui

If the edema is severe, sinking pulse may also be seen.

Treatment method: dispersing wind, clearing away heat, clearing the lungs and promoting water circulation.

Prescription: Yuejiajiashu Decoction.

2. Syndrome of damp toxin invasion

Syndrome: edema of the eyelids, head and face, extending to the whole body, shiny skin, oliguria and red color, body sores, even ulcers, bad wind Fever, red tongue, thin yellow coating, floating or slippery pulse.

Treatment method: Xuanfei, detoxification, diuresis and swelling.

Recipe: Ephedra, Forsythia and Adzuki Bean Decoction plus Wuwei Disinfectant Drink.

3. Water-damp maceration syndrome

Syndrome: edema all over the body, no finger when pressing, short urine, heavy body, chest tightness, anorexia, general nausea, abdominal distension, white coating Feeling greasy, slow pulse, slow onset, and long course.

Treatment: invigorate the spleen and resolve dampness, unblock yang and diuresis.

Prescription: Wupiyin Heweiling Decoction with or without modification.

4. Damp-heat congestion syndrome

Syndrome: edema all over the body, tight and shiny skin, chest and epigastrium tightness, irritability and thirst, short red urine, or dry stool. The tongue is red, the coating is yellow and greasy, and the pulse is rapid or wet.

Treatment method: Dispel dampness and heat.

Prescription: Shuzheyinzi plus or minus.

(2) Yin water

1. Syndrome of spleen yang deficiency

Syndrome: edema for a long time, especially below the waist, and the depression is difficult to recover when pressed , abdominal distension and tightness, anorexia and loose stools, sallow complexion, mental fatigue, tired limbs, short urine, pale tongue, white greasy or slippery coating, slow or weak pulse.

Treatment method: Warm and transport spleen yang to diuretic dampness.

Recipe: Shipi Yin with or without modification.

2. Syndrome of weak kidney yang

Syndrome: edema waxes and wanes repeatedly, face becomes swollen, body is swollen, severe swelling below the waist, depression cannot be felt when pressed, cold pain in the waist, soreness in urine, urination Decreased blood flow, cold limbs, timidity, coldness and fatigue, pale complexion or pale complexion, palpitations, chest tightness, shortness of breath, difficulty lying down, enlarged and distended abdomen, pale and fat tongue with white coating, and thready or slow pulse Powerless.

Treatment method: Warm the kidneys and help yang, transform qi and circulate water.

3. Syndrome of mutual stasis and water stasis
