3. Summarize spleen water (edema caused by spleen dysfunction [1] [2]) as the name of disease syndrome [3]. One of the five viscera edema diseases [3]. One of the ten waters listed in the theory of three causes and one disease syndrome [4]. It refers to edema disease with spleen-yang deficiency, inability to transport, retention of water and dampness, abdominal distension, heavy limbs and short and red urine as common symptoms [1]. "Three Causes and One Disease Syndrome Prescription" is listed as one of the ten waters.

4 Definition of Spleen Water "Terminology of Traditional Chinese Medicine" (2004): Spleen water refers to edema caused by spleen failing to transport water and dampness [2].

Terminology of traditional Chinese medicine (20 10): Spleen fluid refers to an edematous disease with abdominal distension, heavy limbs and oliguria as common symptoms, which is caused by spleen yang deficiency and cannot be transported.

The cause of spleen water is spleen yang deficiency, which can not transport edema caused by water dampness [3]. Huang Di Nei Jing Su Wen Zhi Zhen Da Lun records that "all dampness and distension belong to the spleen".

6 Symptoms of Spleen Water "Syndrome of Water-Gas Syndrome": "People with spleen water have a big abdomen and heavy limbs, but they are bitter and qi-deficient, which is unfavorable for urination." It is often accompanied by symptoms such as eating less and sallow complexion, and its pulse is tidal.

7. Treatment based on syndrome differentiation 7. 1 Spleen edema with spleen-stomach qi deficiency syndrome [1] refers to spleen-stomach qi deficiency, water-dampness stagnation, edema of head, face and limbs, swelling disappearing, loss of appetite, listlessness, shortness of breath, laziness, pale face and listlessness.

Due to long-term eating disorder, the spleen and stomach are weak and delicate, but the face is sallow and the whole body is swollen. Head and face edema in the morning, lower limb edema during exercise, normal or weak stool, frequent urination, thin and greasy tongue coating and weak pulse, which are different from the above edema. This is due to weak temper, loss of qi and inability to transport water and dampness.

7. 1. 1 spleen water spleen-stomach qi deficiency prescription should be used to treat spleen and eliminate dampness, and it is not possible to divide the benefits and damage the qi. Shen Ling Baizhu Powder [Remarks] San 'ao Decoction (Taiping Huimin Hejiju recipe) can be used: Herba Ephedrae, Almond, Radix Glycyrrhizae, or Ramulus Cinnamomi and Radix Astragali, or Fructus Psoraleae and Radix Aconiti Lateralis to warm kidney and help yang. And pay proper attention to nutrition, you can use soybeans and peanuts as adjuvant therapy, which can be cured through treatment.

7.2 Spleen edema with spleen yang deficiency syndrome [1] refers to spleen yang deficiency, insufficient water and dampness, eyelid or whole body edema, abdominal distension below waist, eating less loose stool, short and red urine, sallow complexion, listlessness, cold limbs and dry tongue.

7.2. 1 Symptoms of yin-water and spleen-yang deficiency: facial edema, repeated ups and downs, especially below the waist. It is difficult to recover from depression, which is aggravated after work or in the afternoon, accompanied by fullness, sallow complexion, and? White, listless, short and red urine, clear color, loose stool, pale tongue, white and slippery fur, and deep and thin or knotted pulse. [6][7]

7.2.2 In the analysis of syndromes, yang deficiency, loss of vital qi, and inability to turn qi into water lead to the flooding of pathogenic water in the lower energizer, so the body is swollen, especially below the waist, and can't afford to droop. Dampness is yin evil and sticky, which is more likely to hurt the spleen after fatigue, and edema fluctuates repeatedly, so it is aggravated after fatigue or afternoon. Spleen deficiency leads to abdominal distension, loose stool. Spleen deficiency leads to dull and warm complexion, so the complexion is sallow. The limbs are white, tired and cold. Yang does not turn into qi, and water makes urine short. Pale tongue, greasy or slippery coating, and slow or weak pulse are symptoms of spleen-yang deficiency and stagnant water. [6][7]

7.2.3 Difference between Spleen-yang Deficiency and Water-wet Immersion The difference between Spleen-yang Deficiency and Water-wet Immersion is that excessive water evil leads to yang failure and edema, while spleen-yang deficiency leads to water-wet edema, so it is necessary to distinguish between the primary and secondary importance of evil and right. [7]

7.2.4 Prescriptions for Treating Yin Deficiency, Water Heat and Spleen Deficiency Syndrome [7]

Treatment: Warming spleen and yang, promoting diuresis and eliminating dampness.

Prescriptions such as Shi Pi Yin and Mu Xiang Liu Yin Qi can be used [8].

Can be used for invigorating the spleen [Remarks]: Radix Aconiti Lateralis Preparata, Rhizoma Zingiberis, Atractylodis Rhizoma, Radix Glycyrrhizae, Cortex Magnolia Officinalis, Radix Aucklandiae, Fructus Tsaoko, Areca catechu, Fructus Chaenomelis, Rhizoma Zingiberis Recens, Fructus Jujubae, Poria, Radix Aconiti Lateralis, Fructus Tsaoko, warming yang and dispelling cold; Atractylodis Rhizoma, Poria, Radix Glycyrrhizae Preparata, Rhizoma Zingiberis Recens, and Fructus Jujubae for invigorating spleen and benefiting qi; Pericarpium Arecae, Poria, and Fructus Chaenomelis promote diuresis and dehumidification; Radix Aucklandiae, Pu Chuan, and Pericarpium Arecae regulate qi, and the qi flows smoothly. There is also an edema syndrome. Due to long-term eating disorder, the spleen and stomach are weak and delicate, the face is sallow and the whole body is swollen. In the morning, the head and face are swollen and the lower limbs are swollen. I feel very tired when I can eat, my stool is as usual or thin, and I have frequent urination. Your tongue coating is thin and greasy and your pulse is weak, which is different from the above edema. This is due to weak temper, loss of qi and inability to transport water and dampness. It is advisable to strengthen the spleen and eliminate dampness, but not to divide the benefits and hurt the qi. Shen Ling Baizhu Powder [Remarks] San 'ao Decoction (Taiping Huimin and Jiju Recipe) can be used: ephedra, almond, raw licorice, or cassia twig and astragalus root to replenish qi and yang, or psoralea corylifolia and aconite root to warm kidney and help yang, so as to strengthen gasification. 8 on the classification of ancient edema disease [4]. Past dynasties have different opinions:

1. On the Syndrome of Life and Death of Edema Pulse in the Han and Tibetan Scriptures: "There are ten waters, one is green water, the other is Chishui, the third is yellow water, the fourth is white water, the fifth is black water, the sixth is mysterious water, the seventh is feng shui, the eighth is Shi Shui, the ninth is Lishui, and the tenth is Qi water."

2. "On the Etiology and Symptoms of Edema": "Ten waters, clear water, Chishui, yellow water, white water, black water, suspended water, geomantic omen, Shi Shui, violent water and gas water."

3. Three Causes and One Disease Syndrome Volume XIV: "Heart water, liver water, lung water, spleen water, kidney water, gallbladder water, large intestine water, bladder water, stomach water and small intestine water are ten waters."

9 Edema Edema (edema [1][2]) is the name of the disease [9][ 10]. Published Su Wen's Hydrothermal Theory in Huangdi Neijing. Also known as water vapor [9]. In ancient times, it was called water, water vapor, water disease and water swelling [10]. It is a general term for diseases characterized by skin edema [1]. Patients with fluid retention, submerged skin, causing edema of eyelids, head, face, limbs, abdomen and back and even the whole body, and even severe cases may be accompanied by pleural effusion and ascites [7]. An edema syndrome is a manifestation of systemic gasification dysfunction. The main pathological changes are lung, spleen and kidney dysfunction, unfavorable bladder gasification and edema. According to clinical manifestations, it can be divided into two types: yang water and yin water. Feeling exogenous pathogens, the onset is more urgent, and most of them start from the head and face, and the swelling potential is above the waist. Most of the diseases are located in the lungs and spleen, and most of them are solid and yang water. The onset of internal deficiency is slow, generally starting from tarsal bones, and the swelling trend is below the waist. Spleen and kidney, mostly deficiency syndrome, are yin water [7][6][ 10].

The treatment methods of edema include sweating, diuresis, expelling, strengthening spleen, warming kidney, reducing turbidity and removing blood stasis. If the common methods are ineffective, or there are signs of blood stasis, the method of promoting blood circulation and removing blood stasis can be used [7]. The above methods can be used alone or in combination, and can be selected according to the needs of the disease [7]. Acupuncture has a certain curative effect on the syndrome, and has a good effect on improving symptoms, strengthening physique and reducing recurrent attacks.

Edema in the internal medicine of traditional Chinese medicine can be found in cardiogenic (such as congestive heart failure) edema, nephrotic (such as acute and chronic glomerulonephritis) edema, hepatopathic (such as cirrhosis) edema, endocrine disorder and malnutrition edema in modern medicine [7][6].
