For non-Chinese medicine practitioners, this sentence is meaningless. Similar to some incomprehensible reasons found by feng shui masters.
Refer to the following basic theories of traditional Chinese medicine. Among the five main diseases, those with dull or dull complexion mentioned in Article 5 are mostly kidney-yang deficiency. It is caused by yang deficiency, fire failure, Shui Han failure and cloudy turbidity.
One of the four diagnoses
Inquiry is a way for doctors to observe the external situation of human body with vision, so as to understand the health status and measure the condition. According to the theory of traditional Chinese medicine, man is an organic whole, and the exterior of the human body, especially the face and tongue, is most closely related to the viscera. Local lesions can affect the whole body, while the lesions of qi and blood, viscera and meridians in the body will inevitably be reflected in the corresponding parts of the body surface. Therefore, observing the changes of spirit, color, shape and state can not only understand the overall situation of the human body, but also serve as one of the bases for analyzing the physiological and pathological conditions such as qi and blood and viscera. That is, as "The Collection of Lingshu" said: "If you look at its external response and know its internal organs, you will know its disease."
Inspection is listed as the first of the four diagnoses in TCM diagnostics, and there is a saying that "it is god to know by looking", because human vision plays an important role in understanding objective things. The "Medical Law" says: "Anyone who doesn't know the importance of seeing a doctor, such as a boat boy who doesn't know Feng Shui, is repeatedly drowned, reckless and ill-treated." Therefore, doctors should make full use of visual observation in diagnosis, and pay attention to cultivating and training agile and accurate observation ability in clinical practice and daily life. Through the study of diagnostic knowledge and the accumulation of clinical experience, the observation skills are becoming more and more mature. However, it also has its limitations, so it can't be replaced by other diagnostic methods. It is necessary to combine the four diagnoses to comprehensively understand the condition.
The accuracy of medical treatment is not only related to the accumulation of doctors' knowledge and clinical experience, but also must pay attention to the following points:
(1) Sufficient light to avoid interference: it should be carried out in sufficient natural light as far as possible. If there is no natural light, it should also be done under fluorescent light. Follow-up during the day when necessary to avoid the interference of color light and room temperature.
(2) Full exposure to eliminate illusion: During inspection, fully expose the inspected part so that it can be clearly observed. For individual signs that are inconsistent with the overall condition, we should carefully analyze and eliminate the illusion caused by non-pathological reasons.
(3) Familiarity with physiology and variability: In order to better identify pathological signs, we must be familiar with the normal manifestations and physiological characteristics of tissues in various parts, and compare pathological signs with physiological signs; It is also necessary to be familiar with the relationship between tissues of various parts and viscera and meridians, dynamically analyze and observe with the overall concept, and judge the clinical significance of pathological signs from the perspective of disease development.
(4) Four diagnoses combined with reference, comprehensive judgment: Four diagnoses can't replace observation, and the information of simple observation is not enough and the data is incomplete. Attention should be paid to the close combination of observation and other diagnostic methods, and the four diagnoses should be combined with reference to make a comprehensive judgment.
The contents of the inspection mainly include five parts: the whole body inspection (looking at the spirit, looking at the color, looking at the shape and looking at the posture), the local inspection (looking at the head and face, looking at the five senses, looking at the body, looking at the limbs, looking at the yin and looking at the skin), looking at the tongue (looking at the tongue body and looking at the tongue coating), looking at the discharge (looking at sputum, spitting, defecating and urinating), and so on. Because the content of tongue diagnosis is extremely rich, there is a special chapter to describe it. Although academic interviews are divided into different contents, they need to be comprehensively used in clinical diagnosis without strict distinction.
Section 1 Full body visit
General examination, also known as holistic examination, is that when a doctor examines a patient, he first briefly observes the overall performance of the patient's spirit, color, shape and posture, so as to have a general impression of the cold, heat, reality and urgency of the disease.
Doctors should gradually cultivate their ability to see the whole body in practice, pay attention to the first intuitive impression of patients during examination, and achieve the goal of "feeling in an instant" Especially when the patient is unintentional, it is the manifestation of God's revelation of truth. Therefore, doctors should be calm and estimate the ups and downs of patients' spirits and the severity of their illness through keen observation.
At the same time, the whole body should be in harmony with the spirit. God is the master of form, and form is God's home. Good health makes God prosper, while weak health makes God decline. When the form is inconsistent with the spirit, we should make a comprehensive judgment. For example, if you have been ill for a long time, you will be absent even if you are conscious. The new symptoms are coma and irritability. Although it is rich, it is not a good omen.
First of all, look to God.
God is the general name of human life activities and a high generalization of human life phenomena. God has two meanings. One is "spirit", which refers to the external expression of the functional activities of zang-fu organs; Second, "consciousness" refers to people's thinking, consciousness and emotional activities. The "spirit" here refers not only to the external sign of the functional activities of zang-fu organs, but also to the state of mental consciousness and emotional activities, which is a comprehensive judgment of arrogance and consciousness. Seeing God is a way to judge the illness by observing the overall performance of human life activities.
(A) the principle of looking to God
"Soul Pivot is the God" points out: "The essence is born, and the two essence strive for god". "Ling Shu Ren Ping Jueju" also said: "God is the essence of Shui Gu". It can be seen that the generation of God is closely related to the essence of human body and the function of zang-fu organs. God is born of innate essence, but he must rely on the continuous nourishment of grain essence in the later period. Only when the essence of the first day and the second day of the first day is sufficient, and the qi, blood and body fluid produced by the essence are full, can the function of viscera and tissues be normal and the human body show its spirit. It can be seen that God is manifested through the functional activities of zang-fu organs. Essence is the material basis of God, and God is the external expression of essence. If the essence is sufficient, the body will be healthy and strong, and the disease resistance will be strong. Even if there are diseases, most of them are mild diseases with good prognosis. Deficiency of essence leads to weakness, weak disease resistance, multiple diseases and poor prognosis. Therefore, by observing the ups and downs of patients' spirit, we can understand the ups and downs of their essence, infer the severity of the disease and judge the prognosis of the disease. As "Su Wen Shifts Essence to Qi" said: "Those who gain the spirit prosper, those who lose the spirit die."
(B) the specific performance of God
The theory of traditional Chinese medicine emphasizes "the unity of form and spirit", in which the spirit can only be manifested tangibly and the mental health can flourish. God is the whole embodiment of people's life activities, which is reflected in people's eyes, colors, expressions, postures, etc., and investigating the changes of eyes is the focus of seeing God.
God should pay attention to the first impression when examining patients. God's performance is the most real when the patient is unintentional, so doctors should pay attention to the first intuitive impression when they contact the patient for the first time, so as to be calm, observe carefully and feel immediately. Training can estimate the mental ups and downs of patients and the severity of their illness through short-term observation. At the same time, I hope that God will be in harmony with God. God is the master of form, and form is the home of God, and they are closely related. Health leads to prosperity, while weakness leads to decline. When the form and spirit are inconsistent, we should pay attention to different treatment. For example, physical and mental exhaustion after a long illness is also absent. The new symptoms are coma and irritability. Although it is rich, it is not a good omen.
1. Two eyes
Because the eyes are connected with the brain, and the purpose activities are directly controlled by the mind, the eyes are the external reflection of the mind, so there is a saying that "God hides in the heart and waits for the eyes outside". The theory of soul pivot and great confusion says: "the essence of the five internal organs is to meet the purpose and be refined." The eye is the place where viscera and essence converge, and the visual function of the eye can reflect the ups and downs of viscera and essence, so the focus of looking at God is to observe the eyes. Generally speaking, those whose eyes are full of divine light, wonderful contents, flexible movements and clear vision are gods, and they are images with sufficient viscera essence; Those who have exposed eyes, poor eyesight, ineffective movements and blurred vision are regarded as inanimate, which is a manifestation of insufficient essence of viscera.
Step 2 color
Refers to the color of the whole body skin (mainly the face). The doctor said: "Color is the flag of God, when God is prosperous, it is prosperous, when God is weak, it is declining, when God hides it, it is hidden, and when God shows it, it is exposed." Whether the skin color is moist or dry is an important manifestation of the rise and fall of viscera essence.

Refers to people's spiritual consciousness and facial expression, which is the external expression of the ups and downs of the mind and viscera. The mind is the master of the human body and plays an important role in human life activities. When the mind is normal, people are clear-headed, orderly in thinking, natural in expression and sensitive in reflection; On the other hand, if the mind fails, God will be confused, confused, indifferent and unresponsive.
Step 4: Posture
Refers to human dynamics. The fatness of the body and the difficulty of activities are also external signs of the strength of physical function and the main signs of the quality of God.
When looking at God, we should not only focus on the above aspects, but also make a comprehensive judgment based on God's performance in other aspects, such as language, breathing and tongue pulse.
(3) The classification and judgment of God
Clinically, according to the ups and downs of gods and the severity of illness, it can be generally divided into five categories: ecstasy, absence, false gods and confusion.
1. Deshen
Also known as "God". Its clinical manifestations are flexible eyes, bright eyes, radiant face, implicit, clear mind, natural expression, merciless muscles and sensitive response. It is suggested that being full of vital energy, healthy and full of vigor is a sign of health, or that the disease is easy to be cured and the prognosis is good.
2. Less God
The so-called "listlessness". Its clinical manifestations are glassy-eyed, glassy-eyed, pale, gloomy, listless, slow thinking, lazy speech, soft muscles and slow movements. It is suggested that deficiency of essence and qi and hypofunction are more common in patients with deficiency syndrome or convalescent patients.
absent-minded
Also known as "no god". It is a serious symptom of deficiency of essence or imbalance of pathogenic factors, which can be seen in patients with chronic diseases with deficiency syndrome and excess syndrome.
(1) Absence caused by mental deficiency: The clinical manifestations are dim eyes, dull eyes, dull complexion, dull exposure, listlessness, blurred consciousness, slow response, urine retention in hands, bone loss and emaciation. It is suggested that the essence is seriously damaged and its function is weakened, which is more common in patients with chronic severe diseases and poor prognosis.
(2) Pathogenic pathogens are overwhelming and upset: clinical wheat is now delirious, delirious, following the clothes and touching the bed, pinching the empty line; Or he fell unconscious, and his hands were strong and his teeth were urgent. It is suggested that pathogenic factors are rampant, heat disturbs the gods, and pathogenic factors sink into pericardium; Or liver wind with phlegm, blinding orifices and blocking meridians. They all belong to the serious dysfunction of the body, the imbalance of qi, blood, body fluid, and are more common in acute patients, and their condition is also serious.
4. False gods
People who have been seriously ill for a long time are extremely tired in nature, and those who suddenly have some false expressions of "improvement" for a while are all false gods. If the eyes are dull, suddenly the eyes seem to have light, but they are floating and exposed; Originally, his face was dull, and his face seemed gorgeous for a while, but his cheeks were as red as makeup for a while; I am already stunned or extremely depressed, and suddenly I feel awake and want to see my relatives, but my spirit is restless; Originally, my body was very heavy and it was difficult to move. Suddenly I remembered it, but I couldn't turn it myself. I had no appetite, and I couldn't eat for a long time. Suddenly I wanted to eat, and my appetite increased greatly.
The appearance of false gods is caused by the extreme exhaustion of qi and blood of zang-fu organs, the loss of vital qi, the failure of yin to converge yang, the excess of deficiency yang and the imminent separation of yin and yang. The ancients compared it to "seeing the light again" or "seeing the light again with residual light", which is often a sign before the dying of critically ill patients.
False gods should be distinguished from improved conditions. False gods are generally found in dying patients, and the sudden "improvement" of patients' local symptoms is not consistent with the deterioration of the overall condition, and it is short-lived and the condition deteriorates rapidly. When a serious illness improves, the mental improvement is gradual, which is consistent with the overall improvement, such as increasing diet, moistening complexion and gradually restoring physical function.

5. God's confusion
Insanity Clinical manifestations are often anxiety, fear, mania, apathy, dementia and sudden fainting. , more common in epilepsy, madness, dementia, epilepsy, dirty mania patients.
(1) Anxiety and fear: refers to the symptoms that patients are constantly afraid, anxious, palpitation and short of breath, and dare not be alone in one room. Most of them belong to deficiency syndrome, which is common in humble and dirty patients, and most of them are caused by qi deficiency of heart and gallbladder and mental dystrophy.
(2) Mania: refers to the symptoms such as mania, nonsense, sleeping less and dreaming more, beating people and cursing their mothers, and not avoiding intimacy. Most of them belong to yang syndrome, which is common in madness. It is caused by the stagnation of fire anger, phlegm drinking body fluid and phlegm disturbing god.
(3) Apathy dementia: refers to symptoms such as apathy, dementia, mumbling, dumbfounding, pessimism and disappointment. Most of them belong to yin syndrome, which is common in epilepsy and dementia. Most of them are caused by anxiety and qi stagnation, and body fluid congeals into phlegm, turbid phlegm, or congenital deficiency.
(4) Sudden fainting: refers to the patient suddenly fainting, spitting, eyes looking up, limbs twitching and waking up as usual. It belongs to epilepsy, mostly caused by disorder of dirty qi, rising and falling of liver wind with phlegm, and ignorance.
The clinical significance of insanity is different from that caused by pathogenic factors. Delirium caused by excessive pathogenic factors, such as accompanying clothes and touching the bed, also belongs to delirium, but mainly refers to mental coma, which generally appears in the serious stage of systemic diseases, and the condition is so serious that one has lost consciousness; Delirium here mainly refers to mental disorders, which often recur. There is often no "delirium" in remission, and the condition is not necessarily critical. Delirium symptoms are mainly used as diagnostic basis.
Second, look at the color.
Looking at color, also known as "color diagnosis", is a method to diagnose diseases by observing the color changes of human skin.
Looking at color actually includes observing the color of mucous membrane, secretion and excrement on the body surface, with the focus on the color of the face.
Color diagnosis has a long history. As early as more than two thousand years ago, there were detailed color diagnosis records in Neijing. For example, Su Wenyin and Angelababy said: "Those who are good at diagnosing diseases, look at the color and pulse, and don't talk about yin and yang first." Su Wen's Generation of Five Zangs describes the specific manifestations of the normal color, diseased color and dead color of the five zang-organs, and Ling Shu Wu Se describes the facial parts where the five zang-organs are located in detail. Because of the important value of color diagnosis in clinical diagnosis, doctors in past dynasties attached great importance to color diagnosis.
(A) the principle of color diagnosis
1. The meaning and relationship of color
Looking at "color" actually includes looking at the color of the skin.
(1) Skin color: generally divided into red, white, yellow, blue and black, referred to as five colors. The color of skin can reflect the rise and fall of qi and blood, and also reflect the different nature of diseases and different syndromes of viscera to some extent. The qi of the five internal organs diverges, and the color of the five internal organs can be hidden in the skin. When the internal organs are sick, they can show corresponding abnormal colors.
(2) Glossy skin: refers to the brilliance or dryness of skin color. It can reflect the rise and fall of viscera essence, which is of great significance to judge the severity and prognosis of the disease. Those whose faces are radiant and shiny are those whose zang-fu organs have not lost their vitality, and they have no disease or mild symptoms; Anyone with a gloomy and haggard face is seriously ill because of the decline of viscera essence.
"Four Diagnoses and Micro-diagnosis" said: "The husband's qi originates from the dirty hair, and the color is accompanied by qi." It shows that the skin color of human body shines with the nourishment of essence, which is produced by the functional activities of zang-fu organs, so the luster of skin is the expression of the rise and fall of essence of zang-fu organs. No matter what color you see in clinic, all colors are full of qi, indicating that the essence of zang-fu organs is not aging; If it is colored and lifeless, it means that the essence of zang-fu organs is leaking and failing. Compared with hue, the rise and fall of qi is more important for judging the severity of illness and prognosis. Among the five colors, those who are subtle are qi, and those who are dark are qi, as the book "Looking at the Diagnosis and Following the Classics" says: "Anger cannot be colorless, and color cannot be angry." But in clinical diagnosis, we must also combine the color to make a correct judgment.
2. Diagnostic significance of looking at the face
"Lingshu Zangfu Disease Syndrome" says: "the twelve meridians has 365 collaterals, and its qi and blood are concentrated on the face, so it is deficiency." Because the heart governs the blood vessels, in the face, the three Yang meridians of the hands and feet all ascend to the head and face, especially the stomach meridian of the foot Yangming, which is full of qi and blood and distributed in the face, so the facial blood vessels are rich and proud of the qi and blood of the viscera; In addition, the facial skin is exposed, and its color change is easy to observe. The deficiency of all zang-fu organs and the rise and fall of qi and blood can be reflected by the change of facial color, so the face is regarded as the main part of clinical observation.
3. Face waiting for internal organs.
Treating zang-fu organs on the face is to classify different parts of the face into different zang-fu organs, and diagnose the pathological changes of the corresponding zang-fu organs by observing the color changes of different parts of the face. According to the related discussion in Neijing, there are two specific classification methods:

(2) The classification method of "Su Wen Ci Re" is that the forehead is for the heart, the nose is for the spleen, the left cheek is for the liver, the right cheek is for the lung, and the chin is for the kidney.
The above two methods of facial waiting for viscera can be used as reference for clinical diagnosis. In application, we should mainly observe the overall color changes of patients' faces, supplemented by local color diagnosis. Generally, internal injuries and miscellaneous diseases are mostly treated with five colors of Lingshu, and exogenous fever is mostly treated with five colors of Su Wen's pricking heat.
(2) Normal color and pathological color
Skin color can be divided into normal color and pathological color.
1. Constant color
The color of healthy facial skin is called normal color. Its characteristic is bright and implicit. Bright and moist, that is, the facial skin is bright and moist, indicating that the human body is full of energy, sufficient qi, blood and body fluid, and the viscera function is normal. As Guan Zhen Guan Jing said: "Those who are wise are glorious; Moisturizing cream, the filling of essence and blood. " Recessive, that is, the face is red and yellow, and it is also in the skin, but it is not particularly exposed. It is a sign that the stomach is full and the essence is not leaking. As "Four Diagnostics" said: "If contained, it is a gas reservoir, if exposed, it is a drain."
There are differences due to different physical endowments, seasons, climate and environment. Normal colors can be divided into main colors and guest colors:
(1) Main color: The normal color of human race skin is the main color, also called positive color. The main color is the basic skin color that people are born with, and it is an individual quality, which will remain basically unchanged for life. However, due to race and endowment, the main color is different from red, white, blue, yellow and black. As "Golden Mirror of Four Diagnoses of Medical Cases" said: "The color of the five internal organs, as seen by people with five shapes, remains unchanged for a hundred years, so it is the main color." Most ethnic groups in China belong to the yellow race, and the main color is characterized by faint red and yellow, bright and subtle.
(2) Guest color: normal skin color (especially skin color) that changes slightly due to external factors (such as season, day and night, sunny day or sunny weather, etc.). ) or the difference in living conditions is called guest color. Guest color belongs to the range of normal color, so it still has the basic characteristics of normal color, such as bright and subtle. Its change is not as obvious as the main color, and it is temporary and easy to return to the main color. For example, the face can be slightly green in spring, slightly red in summer, slightly yellow in long summer, slightly white in autumn and slightly black in winter. As the Golden Mirror of the Four Diagnoses of Medical Cases said, "The colors of the four seasons are added with the arrival of the four seasons, so they are not often pushed, so they are also guest colors." Another example is when the weather is hot, the veins are dilated, the blood is full, and the face can be slightly red; When the weather is cold, the veins contract, the blood circulation decreases and stagnates, and the complexion can be pale or greenish. These changes are within the normal range, and we must observe them carefully in clinic to find out and understand them.
In addition to the above changes, people's complexion will also change due to the influence of mood, exercise, drinking, soil and water, occupation and sun exposure. , but as long as it is not blind, it still belongs to the category of normal color.
fall ill
The color displayed on the face when the human body is in a state of disease is called disease color. Diseases are characterized by dullness and exposure. Dull, that is, the facial skin is dry, dull and dull, which is a manifestation of the decline of viscera essence and the inability of stomach qi to flourish. Exposure, that is, the obvious exposure of a certain skin color, is a manifestation of the appearance of sick color or true dirty color. According to the facts, heat syndrome is flushing, that is, disease manifestations; Nephropathy patients have dark exposure, withered and dull, that is, true dirty color exposure. Therefore, the color of the disease can reflect the pathological changes of different properties and different viscera.
Generally speaking, the face of patients with new symptoms, mild symptoms and yang syndrome is bright but still shiny, while the face of patients with chronic pain, severe symptoms and yin syndrome is exposed and dark. The key to observing the diseased color is to distinguish the lustful color from the bad color.
(1) Good complexion: It means that although the patient's complexion is abnormal, it is bright and moist. This shows that the pathological changes are still light, the essence of viscera is not declining, and the stomach qi is superior to the face, which is more common in new symptoms, mild symptoms and yang syndrome. Its disease is easy to cure and its prognosis is good, so it is called lewd. If the patient with jaundice is Huang Liang with orange peel, it is lewd.
(2) Poor complexion: refers to the patient's abnormal complexion and dull. This shows that the lesion is deep, the essence of viscera has declined, and the stomach qi is not superior to the face, which is more common in chronic diseases, serious diseases and yin syndrome. Its disease is difficult to treat and its prognosis is poor, so it is called evil color. If the patient's face is yellow, dark and withered, it is evil.
The examination of facial color in Neijing is recorded in detail, and the complexion is described as "flat, sick, good and evil" by analogy. According to Su Wen's pulse theory and the related discussion in Su Wen's theory of five zang-organs generation, it is determined as follows (Table 2-2).

(3) Five colors are the main diseases.
Disease colors can be divided into red, white, yellow, blue and black, which are found in different zang-fu organs and different diseases. Lingshu five colors think that the five colors belong to the five internal organs, and the corresponding relationship is "green is liver, red is heart, white is lung, yellow is spleen, and black is kidney"; If the five colors reflect the different nature of diseases, then "blue and black are pain, Huang Chi is heat, and white is cold". This method of diagnosing diseases according to the five-color changes of patients' faces is called five-color main diseases, or "five-color diagnosis". Its specific manifestations and main diseases are as follows:
1. Red
Main heat syndrome can also be seen in yang syndrome.
A person who blushes is a real heat syndrome. It is caused by excessive pathogenic heat, accelerated blood circulation, facial pulse expansion and qi and blood filling.
