Don't write your last name yet. Select A2:D2, and press and hold the filling handle to pull to the right. Pull as many columns as you need.

Then enter: =A2+ 1 in cell A3.

Enter: =C2+ 1 in the C3 cell.

Select A3:D3, and press and hold the filling handle to pull to the right. Pull as many columns as you need.

Select line 3 (just click the line number)-Copy-Select lines 4 to 32-Paste.
