(2) District and county organizations
All districts and counties need to set up corresponding leading groups, and set up several working groups to be responsible for organizing and implementing the physical education examination in their own districts and counties.
Second, the examination object and examination registration
(A) the examination object
Anyone who takes the Xi 2065 438+05 junior high school graduation exam (that is, promoted to ordinary high schools, vocational high schools and secondary specialized schools; Candidates (including former students, candidates who returned to our city to take the senior high school entrance examination in different places and all junior high school graduates) need to take a unified physical examination; Candidates who do not take part in the unified physical education examination for no reason shall not be promoted to institutions of higher learning.
(2) Examination registration
Physical education examination registration and technical secondary school registration are carried out simultaneously, numbered uniformly, and candidates take unified physical education examinations in the registration districts and counties. The unified examination items of physical education examination are selected by the candidates themselves in the online registration process, and the registration information may not be modified after confirmation.
Three, the examination content, performance evaluation and examination methods
The physical education examination consists of the usual examination and the unified examination, with a full score of 50 points. Among them, the normal assessment score is 15, and the unified examination score is 35. The test results are presented in four grades: A, B, C and D, with 50-45 as excellent (a), 44.9-37.5 as good (b), 37.4-25 as qualified (c) and below 24.9 as unqualified (d). The original score is included in the total score of the senior high school entrance examination.
(A) peacetime assessment
1. Normal assessment of fresh graduates: 5 points for each academic year, full marks for three years 15 points. Among them, physical education class scored 3 points per academic year and scored 9 points in three years; The test score of "National Students' Physical Health Standard" is 2 points per academic year, with a full score of 6 points for three years. The usual assessment is organized and implemented by the school, and the results of freshmen's usual assessment should be reported to the district and county admissions examination institutions before April 3.
2. Previous students' usual assessment: 80% of the full marks (i.e. 12) are directly recorded by the district and county admissions examination institutions.
3. The fresh candidates studying in different places and across counties in our city shall be issued by the school with the usual assessment results, and reported to the district and county admissions examination institutions for examination and filing.
(2) Unified examination
The unified examination is organized and implemented by the enrollment examination institutions of all districts and counties, and the time is: April 5, 2065438+April 30, 2005.

2. Examination method: According to the Detailed Rules for the Implementation of the Unified Physical Education Examination for Junior High School Graduates Xi 2065 438+05 and the Workflow of the Unified Physical Education Examination Xi 2065 438+05 (hereinafter referred to as the workflow) (see annex 1 2). If the electronic test instrument project has been equipped, it shall be implemented in accordance with the prescribed workflow. All items of the unified examination must be organized and implemented under video surveillance.
3. Scoring method: Check the individual scores according to the "Scoring Table of Unified Examination Contents and Achievement Transformation Standards for Junior High School Graduates in Shaanxi Province" (see Annex 3).
The conversion formula is: total score 35 =(65438 items +0 points +2 items +3 items +4 items) ÷4×35%.
4. Test equipment: except volleyball skills, electronic test instruments are used uniformly.
Fourth, the examination rules and requirements
(1) Examination institutions
All districts and counties shall set up examination institutions to be responsible for the organization and implementation of unified physical examination.
The examination institution has 1 as the examiner, and 1-2 as the deputy examiner, and is the director and deputy director of the district and county admissions examination institution (see Annex 5 for responsibilities). The examination institution consists of examination group, medical security group and score information management group.
1. Responsibilities of the evaluation team: formulate the examination procedures; According to the requirements of examination items, standardize the arrangement of examination rooms and strictly check the examination equipment and facilities; Appraisers have a reasonable division of labor and solid business training; Organize and implement the review of various projects; Deal with the examination problems in the examination process; To rule and stop candidates from violating discipline and rules. The head of the evaluation group is a PE teacher with a senior title.
The assessment group consists of an assessment group and an examination group, whose duties are:
(1) Sign-in team: check whether the candidates' clothes meet the requirements; Urge candidates to prepare for activities; Register and group the candidates who take part in the test according to the requirements (grouping must be carried out according to the principle of the same class, the same gender and the same project in each group); According to the workflow, the registered candidates will be taken into the examination room to queue up.
(2) Examination Group: With the "Xi 'an 20 15 Entrance Examination for Junior High School Graduation Physical Education Examination" (see Annex 4; Whether the photos on the admission ticket are consistent; Inspect according to the specified inspection method; After completing the examination with electronic examination instruments, a unified examination result report will be printed immediately and signed by the invigilator of the candidates; Volleyball skill test items, the test results are registered in the corresponding column of the admission ticket, and the examiner confirms the signature (or seal), the examinee inspector confirms the signature, and the examinee informs the signature; Ensure the safe use of instruments in the process of unified examination; Maintain the examination style and discipline; Immediately verify and answer the candidates' on-site complaints, and report the difficult questions to the marking team leader in time.
2. Responsibilities of medical security group: responsible for medical rescue, security and logistics support of test sites during the unified examination; Equipped with corresponding medical first-aid equipment, first-aid drugs, professional ambulances and corresponding support equipment; Properly handle the sudden illness, injury and physical discomfort of candidates or staff in the examination room; Closed unified examination room, shielding radio signals.
The medical security team consists of 1 team leader, more than 2 professional medical staff with clinical first aid experience, and several full-time security guards.
3. Responsibility of the score information management group: print the admission ticket; After the examination, the "admission ticket" filled with examination results shall be recovered, saved, backed up, uploaded and managed, and the results shall be imported, synthesized and verified to ensure the safety of information equipment and data.
The head of the group is the director of the district and county entrance examination institutions, and there are 1 full-time performance manager (by the examination cadres of the district and county entrance examination institutions) and 1 information manager (by the information management cadres of the district and county entrance examination institutions).
(2) Setting of test sites and examination rooms
1. Test center setting: Candidates must take the test at standardized test centers, and must be equipped with outdoor unified testing plastic track and field (with a circumference of not less than 200m and no less than 6 circular runways) and indoor unified testing sites, which are easy to close, and equipped with wired telephone, broadband network and radio signal shielding equipment.
The test center needs banners, road signs, infirmary, waiting room, leading teachers' lounge, etc. , and posted a work flow chart, unified examination area plan, examination rules and candidates' instructions, etc.
2. Examination room setting: unified examination sites and venues must be free from potential safety hazards, and the venues, places to be examined, examination sites and toilets should be recorded. It can be arranged reasonably and loosely, and there are safe passages between venues (including venues of various projects). The site should have the conditions to set up medical points and drinking water supply points. Unified test sites must be fully enclosed management and radio signal shielding.
(three) the use of test instruments and other equipment
1. All items except volleyball skills in the unified examination use electronic test instruments.
2. All items in the unified examination must be monitored and recorded by video with unified equipment during the examination (video materials must be kept for more than half a year).
3. After the exam is completed, every exam content using the electronic exam instrument will be printed to the candidates on the spot.
4 test sites set up special monitoring and inquiry terminals for invigilators and staff. The right to use the terminal must be set to query only, and no data can be modified or deleted.
5. Before the examination, all districts and counties should contact the instrument manufacturers according to the requirements of the unified examination, complete the setting of the examination system software in time, and maintain, debug and test the examination instruments. Instruments that have not obtained the qualified test report shall not be put into use.
6.3.30 days ago, all test sites in the city should conduct a mock exam for all items. Ensure that all test equipment is running normally and all test sites have no potential safety hazards, and report the simulated test situation to the Municipal Education Testing Center in writing before April 3.
(4) Fill in the admission ticket
1. Admission examination institutions in all districts and counties print and issue admission tickets according to the specified time (the printing software and format are in accordance with the unified template of the Municipal Education Examination Center). Each school must arrange a special person to carefully check the name, admission ticket number, unified examination items, bar codes and photos of each candidate on the admission ticket (if any wrong information is found, it should be fed back and corrected to the district and county entrance examination institutions in time), and affix the seal of the school or district and county entrance examination institutions after confirmation, and arrange a special person to strictly manage and save the admission ticket to prevent loss.
2. Each registration school should issue an admission ticket to each candidate before the unified examination, and candidates must take the unified examination with the admission ticket.
3. In the process of filling in the column of the unified examination results, if there are clerical errors or incorrect results, the admission ticket should be corrected in time and signed and sealed. When the admission ticket needs to be replaced, the evaluation team leader must fill in the replacement reason in the upper right corner of the invalid certificate and sign it, and attach the invalid certificate to the new certificate.
(5) Data management
1. After all the items in each examination group are tested, the recovered admission ticket must be kept by the grade manager, and the admission ticket must be sealed in the confidential cabinet in the confidential room. The keys to the secret room and the secret cabinet shall be kept by the competent director and the performance manager respectively. 24-hour video surveillance is implemented in the security room, and it can only be closed after all the results are reported. The video materials will be kept until 10 days after the results of the senior high school entrance examination in this city are announced.
2. Set the operation authority of unified inspection instruments and working computers, and implement the responsibility to people. The information administrator must complete the work of machine transmission under the supervision of the personnel of the supervision department and the performance administrator.
3. Computers with unified examination results data shall be kept by more than two people during the unified examination (including lunch and rest time), and shall not be taken out of the unified examination place, and two people must be present when operating the computer.
4. All districts and counties shall report the comprehensive results of physical education examination to the Municipal Education Examination Center in the form of CD before May 15. (The CD is in triplicate, and the name of the district and county and the signature of the person in charge shall be indicated on the CD. )
5. After the unified examination, all districts and counties must sort out the admission tickets of all candidates according to the order of school numbers from small to large, and seal them in the confidential room of the districts and counties until half a year after the end of the cultural examination. The Municipal Examination Supervision Team (the Supervision Office of the Municipal Education Bureau, the Health Department of Sports Arts, and the Municipal Education Examination Center) will conduct spot checks.
(6) Exemption from testing and postponement of testing
1. No test.
Candidates may apply for exemption from physical examination due to illness (serious diseases in major organs such as heart, lung, liver and kidney), abnormal physical development, limb disability and deformity (those who seriously affect their functions).
Among them, candidates with physical disabilities reaching Grade I, II and III disabilities, with the "People's Republic of China (PRC) Disabled Persons' Card" and the school certificate, will be included in the total score of the entrance examination after being approved by the district and county entrance examination institutions. The physical examination scores of other exemption candidates are included in the total enrollment scores of colleges and universities according to 35 points.
Candidates who apply for exemption should apply for exemption at the time of registration with proof materials (disability certificate or hospital diagnosis certificate at or above the county level, school certificate, candidate application). The enrollment examination institutions in all districts and counties must be strictly examined and catalogued before April 3, and reported to the Municipal Education Examination Center.
Candidates who have (or have had) major organ diseases or motor dysfunction and want to take a unified physical examination must provide a certificate from a hospital at or above the county level in advance, and they can take the exam only after being signed by their parents.
2. Slow down the test speed.
Candidates who cannot attend the unified physical examination on schedule due to sudden illness (or injury), female students' physiological period and other reasons may apply in writing to suspend the examination, and the examination can be suspended only after being approved by the district and county entrance examination institutions, but the examination must be completed within the time specified by the district and county unified physical examination institutions, and the district and county entrance examination institutions do not have a make-up room.
(7) Avoidance system
Third-grade teachers are not allowed to take the unified physical education examination; Examination staff must avoid the unified examination items that have the reference of candidates in our school; Personnel who have immediate family members to take the exam may not take the unified physical education exam.
(8) Data report
The enrollment examination institutions in all districts and counties shall report the group examination work plan, examination work arrangement, exemption list, simulated examination results, books and post responsibility personnel list to the Municipal Education Examination Center before April 3. Before May 15, the summary of the results of the unified physical education examination and the CD-ROM of the synthetic results of the physical education examination will be submitted to the Municipal Education Examination Center.
(9) Pre-examination education
All junior high schools should conscientiously carry out pre-test education and publicity activities, publicize policies and regulations through parent-teacher conferences, class meetings and other forms, correct the examination style and discipline, and do a good job in pre-test counseling. At the same time, it is necessary to carefully verify the physical health of each candidate. Remind candidates who are not suitable for unified medical examination to go through the formalities of extension or exemption in advance according to regulations.

The members of the supervision team are composed of relevant personnel from the Supervision Office of the Municipal Education Bureau, the Stability Office, the Sports and Arts Department, the Municipal Education Examination Center and the Education Institute. During the examination, the supervision team will go to the 14 examination area for supervision and inspection.
All districts and counties must establish and improve the supervision mechanism of physical education examination and supervise and inspect the whole process of examination. The first is to standardize the implementation of the candidate supervision system. Candidates' inspectors are set up in strict accordance with the requirements to ensure that candidates' inspectors supervise the whole process of the unified examination of physical education examinations in this group and earnestly exercise their supervisory rights. Second, strengthen the supervision of functional departments. The member units of the county work leading group must include supervision institutions or departments, which should supervise all aspects of the examination. Third, give full play to the positive role of social supervision. The county education bureaus, enrollment examination institutions and schools should attach great importance to all kinds of safety problems existing in the unified examination of physical education examinations, formulate safety work plans to prevent and respond to emergencies, establish a responsibility system for safety work, and designate a person to be responsible. The examination center is fully enclosed. Invigilators and staff wear signs to enter the examination room, candidates enter the examination room with the admission ticket, and irrelevant personnel are not allowed to enter the examination room. Schools with different entrance examination institutions, test sites and students in all districts and counties should implement their responsibilities from the aspects of pre-test education, physical examination of candidates, round-trip transportation, food hygiene, medical rescue, safety inspection of equipment and examination rooms, eliminate hidden dangers and improve the level of emergency response. All districts and counties and related schools should establish the guiding ideology of "safety first", formulate detailed safety plans for physical education examinations, and implement responsibilities. All districts, counties and schools should do a good job in adaptive training for candidates before the exam, earnestly do a good job in safety education for candidates, and designate a person to be responsible for the safety of candidates during the exam. Check the safety of the vehicles that pick up candidates. driving without a license, drunk driving and overloading are strictly prohibited. Conscientiously implement the pre-test notice, physical examination and medical security. Before the exam, you must explain the exam rules and safety requirements to the candidates.
